Can a Doctor Tell if You Smoke by Looking at Your Throat?

i smoke occasionally (about 1 or 2 cigarettes a day) i have to have a physical and i am worried that the doctor will be able to tell that i smoke. so if a doctor gives me a physical and checks my throat will he be able to tell if i smoke? i also live with a smoker, so would it be possible for the doctor to think that it is the second hand smoke and not actually me smoking? please help and dont preach to me about the dangers of smoking. i know it all already.
